IMPORTANT: Please read this document carefully. It is a condition of your use of our services that you comply with the terms and conditions outlined below.

Client Disclaimer

I give consent for EP Clinic Noosa to record and store notes, medical information, or other pertinent information related to services received.

EP Clinic Noosa’s privacy policy complies with all Commonwealth, State, and local privacy laws. This privacy policy can be viewed on request.

I authorize EP Clinic Noosa to release any medical information, verbally or in writing, as is necessary, relevant to the management of my chronic condition(s). I understand that EP Clinic Noosa does not accept responsibility for lost, stolen, or damaged valuables, cash, or personal items.

I acknowledge and agree that:

(i) I am aware that the facilities and services offered by EP Clinic Noosa involve risk, including but not limited to the risk of bodily injury.

(ii) I have and will provide all relevant information regarding my medical history and current health status.

(iii) I am making use of the facilities and services of EP Clinic Noosa of my own free will.

(iv) I understand that any gym or clinic that EP Clinic Noosa conducts services in is not liable for any injury, theft, loss, or damage.

(v) I assume all risks associated therewith.

Accounts and Payment

I understand that payment for my account is my responsibility, and all accounts must be paid on the day of service. If any account is not paid within (30) days, I agree to pay all expenses incurred by you in pursuing recovery of overdue amounts, including (but not limited to) legal fees and administrative costs. Please note that the responsibility of accounts is yours until such time that your claim is accepted by the insurance body. Accounts will be held in abeyance until such time as a decision is made.

Cancellation and Late Arrival Policy

The following procedures have been put in place to alleviate the disruption to this clinic and to other clients.

(i) EP Clinic Noosa has a 24-hour cancellation policy that applies to all individual and group session bookings. Please ensure you let us know in advance if you are unable to attend and reschedule accordingly. (An SMS reminder 24 hours prior to your appointment is sent to you so you can notify us via reply Yes or No of your attendance status). If you do not provide adequate notice or fail to attend your session, 100% of the scheduled fee will be charged. Please be aware that this fee will not be covered by your private health insurance, work cover claim, DVA claim, or Medicare claim. EP Clinic Noosa will take into account extenuating circumstances.

(ii) If you are unable to attend your scheduled appointment, the clinic will, when able, provide you with an alternative appointment time on your scheduled day of service.

(iii) Clients who either arrive late or miss an entire appointment will be charged 100% of the scheduled fee.
